One Webpage with Data Collection Form

I hope you can help. I’m wanting to put together a single page website with a data form. One we have already done please click here: www.taxfriend.co.uk



As you can see the webpage is very basic but has to look professional. The data form should look clean and professional as well and I like the idea of flash animation or pictures on the page. The data form will be run by us so there is no need for any work your end just the design Work. We shall give you all the copy that is needed for the web page and logo (and the companies website if needed). All the web page needs to do is to capture data and put into a MySql database, anything like this needs to be in php.



I have already had quotes for this work already from other companies and the cheapest I’ve had is $40.



Can you do the above work for around the same costs? If so and the work is of high standard then there will be more work for your company, as many as 8-10 a month.
